Vernon County CHART

(Community Health Assessment Resource Team)

The Vernon County CHART Coalition was formed in February, 1997, for the purpose of assessing the health status of Vernon County, prioritizing health issues and needs, and planning for a community strategy to address those issues.

The coalition is made up of persons representing area agencies, organizations, and businesses, as well as individuals, who are interested in a healthy future for Vernon County. Anyone who is interested is welcome to attend CHART meetings.

CHART meetings are held monthly to increase awareness of area activities and resources, to work on the community plan, and to increase interagency cooperation.
